Leave of Absence Form

If your child is going to be away from school please complete and return the Leave of Absence form as soon as possible.

We aim for 100% attendance at Holly Meadows.  Please avoid term-time holidays where possible.                                     

Please note that as per our attendance policy  holidays are not routinely authorised during term time.
